You and your family can earn extra rewards – in addition to all the benefits you already receive. CountyCare members qualify for a variety of rewards and other benefits that promote a healthy lifestyle. CountyCare members who have earned rewards can expect their Rewards Card to be mailed in the coming weeks.
The CountyCare Rewards Card Program lets you earn rewards to spend on lots of items at participating stores.
How it works:
When you or your children go to the doctor for certain services, CountyCare will automatically send you a CountyCare Rewards Card in the mail. The card will have a credit on it based on the services you or your children received. CountyCare members who have earned rewards can expect their Rewards Card to be mailed in the coming weeks.

- Continue seeing your doctor, and earn more rewards! CountyCare will automatically credit your existing card.

| Annual Health Risk Screening $15 reward
CountyCare will give each member a $15 credit on their Rewards Card once a year for completing a Health Risk Screening. Call Member Services to connect with your care coordinator and complete the screening.
| Healthy Moms Program $10/$25 reward
Pregnant members should see their doctor regularly, including after you have had the baby.
| Well-Child Visits $10/$25 reward
You can get rewards for taking your baby to the doctor in their first 15 months.
| Childhood Immunizations $10 reward
You get rewards for taking your baby for vaccinations before they turn two years old.
| Breast Cancer Screening $25 reward
Women ages 40 to 74 can earn a $25 reward each year when they get their mammogram.
| Managing High Blood Pressure $25 reward
If you have high blood pressure, CountyCare will give you a $25 reward if you fill your prescriptions and take your medicine as your doctor recommends for at least six months in a row.
| Managing Diabetes $25 reward
When a member with diabetes goes for their annual PCP visit and gets their blood tests and urine screens, CountyCare will add $25 to their CountyCare Rewards Card.
| Eye Exams for Diabetics $25 reward
When a member with diabetes goes for their annual eye exam with a CountyCare eye care professional, CountyCare will add $25 to their CountyCare Rewards Card.
| Care Management $25 reward
Members with high-risk conditions are eligible to enroll in our Care Management program to meet with a Care Manager regularly. Members will be rewarded $25 for enrolling and completing a care plan with their Care Manager.
| Behavioral Health Follow-up Visits $10/$20 reward
Members who follow-up with their doctors after ER visits or inpatient stays for behavioral health will receive the following rewards.
| Follow-up After Hospital Admission $20 reward
$20 for seeing your doctor within 14 days after an inpatient hospital stay.

| Free Diapers
When a child under the age of two is on schedule for shots, CountyCare will automatically mail a coupon for a Jewel-Osco Baby Basics Jumbo Pack of diapers each month. The coupon can be used at any Jewel-Osco location in the Chicagoland area. The cashier will verify your coupon when you check out. Click here for more details.
| Free LASIK Eye Surgery
CountyCare offers LASIK eye surgery for eligible members at no cost. LASIK is a surgical procedure that is used to correct vision problems by re-shaping the corneas. To be eligible for LASIK a member must be between 21 and 50 years of age and in good general health. A Member must have an eye exam and a refractive exam to determine if he or she is a candidate for surgery. Click here for more details.

If a pregnant member completes at least 13 prenatal visits, they will receive a free Sleep Safe Kit delivered to their home within the first few months after giving birth. The kit includes:

Children enrolled with CountyCare are eligible for a free car seat. Parents can call to get them at the location listed below.

Members age 3 through 16 can receive a new book in the mail every three months by calling member services and requesting one. Kids ages 5 through 16 will receive a retail $10 Target gift card along with their book.
| Free Home Pregnancy Test
Female members of child bearing age can call member services and request up to one test a month. It will be mailed to the address they provide.
| Weight Watchers Membership
CountyCare members 13 and older get free vouchers for Weight Watchers meetings near your home. Call Member Services to request and we will mail them to you.
| Free Cell Phone
The program provides free minutes for calls to CountyCare Member Services.
