Pay-for-Performance Program

Due to the HealthChoice Illinois managed care expansion contract, implemented on January 1, 2018, HEDIS year 2019 (HY19) data collected in calendar year 2018 (CY18) will be considered baseline data to be used for comparison in future measurement years.  Neither the State nor CountyCare will offer formal Pay-for-Performance (P4P) programs for HY19. The State’s and CountyCare’s formal P4P programs will resume in HY20 (calendar year 2019).

Please continue to utilize Vision to identify members who need services captured by HEDIS measures.  CountyCare providers are expected to provide high quality care, follow practice guidelines posted on CountyCare’s website, and promote member self-management through getting needed care.  In addition, care provided this calendar year will often support next year’s HEDIS measure compliance, as measurement follows clinically appropriate look back periods.

Stay tuned for updates in the near future about pay-for-performance programs support by HealthChoice Illinois (Illinois Department of Healthcare and Family Services) and CountyCare Health Plan.  CountyCare welcomes provider input about future P4P initiatives.  Please forward your input to
